$100K starting salary
$5,000 sign-on bonus with additional bonus opportunities in Year 1

Demonstrates leadership and provides direction to members of the client care team through case management of client home health episode of health care. Case management of client services includes; intake and coordination of a referral, initial in home evaluation, establish the client's individualized treatment, and collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care.
Facilitates active and effective communication with team members as demonstrated through leadership of client case conferences, staff education or in services, and timely clinical decisions which provide guidance in the delivery of client care.
Collaborates with leadership/management team and office staff to facilitate timely office processing of paperwork and personnel information.
Actively participates in Agency quality improvement activities.
Promotes the Agency and services to the public and participate in physician and community education related to services provided by the Agency as registered.
Degree in Nursing required.
Registered Nursing licensed in the Commonwealth of Virginia.
Must have home health care experience.
Must have 3-5 years experience in hospital, long term care facility or other related healthcare facility.
Supervisory experience required.
Must have demonstrated experience in performance management and effective leadership.
Must possess strong written and oral communication skills; and ability to maintain confidentiality client information.
