Performs duties as a patient navigator. Coordinates with other navigators assigned to underserved areas, facilitating entrance of eligible participants into the outreach program, and creating an efficient flow process for screened patients for treatment as needed. Recognized for clinical expertise in psychosocial assessment of patients diagnosed with cancer and coordinates access of new patients into the Geisinger Healthcare System. Responsible for the intake of referrals and subsequent coordination across the continuum of cancer care. Serves as the liaison between the patient and caregivers prior to initial evaluation, during treatment and surveillance as indicated by the disease specific team. Coordinates utilization of resources in the most cost-effective manner to meet patient's ongoing psychosocial and continuing care needs.
Works under the administrative direction of the Operations Manager/Director of Operations and the clinical direction of the physician members of the multidisciplinary team.

EDUCATION AND/OR EXPERIENCE:
Master's degree in Social Work with valid Pennsylvania licensure to practice required.
Knowledge of cancer services, preferably to include surgery, radiation, and chemotherapy.
Knowledge of community resources, state and federal assistance programs.
Must participate in continuing education activities to maintain and enhance knowledge.
Minimum of two years of clinical experience required, preferably in an oncology related field
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in a clinical environment. Minimal travel between Central hub Geisinger Health System facilities is required.
