JOB SUMMARY:

As a Cardiovascular (CV) Quality Improvement Consultant RN, this position provides clinical expertise and works with medical teams to improve performance and patient outcomes, achieve transformation and sustainable change which supports the System strategy of providing Value-Based HealthCare (VB HC) in accordance with regulatory statutes, policies and procedures.

WHAT IS EXPECTED (ESSENTIAL FUNCTIONS):

• Serves as an internal Quality Consultant for pursuing Value-Based HealthCare initiatives within the Facility, and/or small to medium scale projects throughout the System. 
• Facilitates, coordinates, and provides project management for quality improvement teams, strategic planning sessions, as well as other organizational meetings as requested and needed. 
• Coaches and mentors clinical staff and management in the provision of quality improvement and regulatory issues.
• Develops multidisciplinary indicators and identifies opportunities for improvement, in collaboration with teams & leaders, to monitor and evaluate the quality of care and services provided to patients based on evidence-based / value-based practices, regulatory and accrediting agency requirements, and data analysis of high risk, high volume and/or problem prone processes.
• Prepares regular status reports on the progress of work teams to various internal constituents to keep them apprised of progress, potential barriers, lessons learned, and related topics to include the timely submission of monthly reports for all strategic improvement priorities.
• Works with applicable staff to utilize appropriate audit strategies, electronic data extraction, expert observation, surveys, focus groups or other tools to obtain relevant information to improve care and quality of service.
• Participates in design, development, and implementation of community learning forums.

• Knowledge of Cardiovascular / CV acute-care
• Knowledge of the tools and techniques of quality performance measurement. Knowledge of the trends, developments, and research in providing Value-Based HealthCare.
• Facilitation, coaching, consulting, influencing, and project management skills.
• Strategic thinking skills.
• Proficiency in the use of computers and related software applications.
• Verbal, interpersonal and written communication skills.
• Ability to coach and mentor staff in the provision of quality improvement and regulatory issues.
• Ability to work collaboratively and effectively with a wide range of constituencies and across organizational lines.
• Ability to define project management processes, tools, and templates as required to drive initiatives.
• Ability to navigate strategic and operational challenges to achieve objectives.
• Ability to identify potential issues that will impede success and recommend and implement corrective action

QUALIFICATIONS

- EDUCATION – Bachelors Degree, preferably in Nursing

- EXPERIENCE - A minimum of 5 years of experience required

- PREFERRED QUALIFICATIONS – Registered Nurse, Masters Degree, and clinical experience in Cardiovascular / CV acute-care
