You Will Be Responsible For:
Provides nursing triage services and health advice for patients and managed care members over the phone. Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and, as possible, obtaining vital sign and mental status information
Documents immediately patients' questions or concerns in a clear and concise clinical manner. Tracks questions/concerns/calls in appropriate database
Determines urgency of seeing the patient based on brief assessment and on familiarity with a patient’s condition and history. Confers with physician as needed. May give physician referrals as needed
Sends those with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to ER immediately or arranges for ambulance
Sets up appointment for patients who do not need to go to ER but need to see a Physician or arranges for a same day appointment
Acts, when designated, in “Ask a Nurse” capacity, handling routine information and education requests from patients. May provide appropriate home health advice to those patients who do not need to go directly to the ER.
Collaborates with Social Worker when members and their families are in need of financial counseling or other social needs.
Provides feedback to the multi-disciplinary team as necessary regarding the trending of any issues identified and collaboratively works to develop action plans to improve processes.
Provides ongoing call review and education for phone agents
Completes transition of Care Management calls to members within 48 hours of a hospital discharge, and schedules follow up appointments within 5 business days of discharge.
Serves as a member advocate and enhances a collaborative relationship between the physician and multidisciplinary team with the member and family to ensure needs are met.
Provides education to patients for chronic and acute illnesses
Assists with creating new employee education material and continuing education materials used in the Physician Practices for Triage, chronic and acute illnesses
Required to train new employees who may assist with Triage in their role within Well 65+
Reviews Well 65+ policies and procedures periodically and makes recommendations to assure continued compliance with current regulations
Assists in the development and implementation of written policies and procedures that govern the operations of the Triage process and education
What You Will Need:
Registered Nurse (RN) degree (BSN), required
Minimum five (5) years of experience as RN, required
MSN degree, preferred
One (1) to two (2) years of telephone triage experience, preferably in medical practice setting, preferred
Prior medical surgical experience, preferred
Current valid State of Florida or multi state license as a Registered Nurse, required
CPR/BLS Certification, required
Excellent communication skills particularly listening
Critical thinking skills and sound clinical decision maker
Proficient in Chronic Care Management
A supportive attitude toward our patient population of older adults
Proficient in computer, Microsoft office, EMR and telephone etiquette
Familiar with standard concepts, practices, and procedures within a medical subspecialty
Demonstrates leadership to other proximal clinical support team members
Requires good communication skills both in writing and verbally
The Lead Registered Nurse will be responsible for Phone Triage education and training for new employees. Provide patient education for chronic and acute illnesses. Reports to the Sr Manager of Clinical Operations and works within the Access Center collaborating with the Manager of Access & Referrals. Working with an ambulatory outpatient office setting, provides triage (sorting/prioritizing patients) telephone service to ensure prompt identification of patients with high-risk conditions. May answer calls for the majority of the day, refer patients, schedule appointments, or provide appropriate education/advice to callers. Responsible for providing service to all patient and family/caregiver telephone calls received and coordinates/dispatches appropriate level of care to patient. The Lead RN – Phone Triage supports and coordinates patient requests to ensure that the appropriate care is given in a timely manner as deemed necessary.
