You Will Be Responsible For:
Demonstrates, through behavior, AdventHealth’s service standards.
Observes, evaluates, reports and educates practice staff regarding systems and processes, including but not limited to: patient flow and experience, provider and staff experience. Implements best practices of population health performance processes and human and technological resource utilization.
Responsible for deploying quality initiatives and evaluating opportunities for process improvement within providers practices. Serves as a quality liaison between operational team and Medical Directors to maximize practice performance.
Visits physician practices to assess and assist in process improvement opportunities within office workflow and physician and staff education.
Works closely with the AHPN-CF and IHS Leadership in deploying and operating plans for the initiatives identified by the physician governance and the population health leadership.
Understands the AHPN-CF quality and utilization measures and assess and educates other providers in the network on best practices of other top performing thresholds.
Responsible for identifying potential open care gaps and capturing necessary data to satisfy these measures.
Outreach to engage consumers using motivational interviewing and schedules wellness and care pathway visits.
Assists in chart prep and review for accurate documentation of burden of illness and identify opportunities to improve medical care for quality measure success.
Develops routine schedule of in-office visits to address quality and utilization opportunities in addition to auditing practice follow through of Service Delivery Expectations

Education and Experience Required:
High School Diploma or equivalent
Knowledge and Skills Required:
Strong organizational skills, self-directed, and ability to manage multiple priorities/projects
Effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of leadership, physicians, and staff
Proficiency in Microsoft Office Suite
Excellent communication, interpersonal skills and ability to lead, facilitate, consult, research and educate
Ability to learn AHPN-CF population health management tool(s) to improve success and identify quality and utilization opportunities to drive network performance
Ability to manage and work with a diverse group of individuals, while striving for excellence
Ability to accept responsibility for actions and results, with a drive to achieve and outperform, while continuously looking for improvements
Problem solving skills with the ability to think creatively

The AdventHealth Population Health Practice Enhancement Coordinator is responsible for deploying quality initiatives and evaluating opportunities for process improvement within providers practices. This includes but is not limited to, consumer outreach and assisting in scheduling for appointments with the AdventHealth Physician Network – Central Florida (AHPN-CF) providers, navigating different electronic medical records to assist in closing care gaps, prepping charts for accurate documentation of burden of illness, and visiting physician practices to assess and assist in process improvement opportunities within office workflow and physician and staff education. The Coordinator works closely with the AHPN-CF and IHS Leadership in deploying and operating plans for the initiatives identified by the physician governance and the population health leadership. The Coordinator will execute on strategic initiatives related to quality, access, and accurate documentation of burden of illness to achieve the stated growth plans for AHPN-CF. Collaborates with various departments across AdventHealth to collect feedback and support. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
