The role you’ll contribute:
The Central Florida Division (CFD) Program Director of Post-Acute Care Collaborative is the leader responsible for the development, implementation and evaluation of prioritized Post-Acute Care Collaborative initiatives across AdventHealth CFD include North, South and Polk markets. The scope of Post-Acute Care Collaborative includes Skilled Nursing Facilities (SNF’s) and Home Health Agencies including owned entities, Joint Venture and community partners. The Program Director collaborates with local post-acute and community leaders to develop and deploy standardized policies, procedures and processes to achieve the AdventHealth vision. The Program Director is also responsible for the development and deployment of standard education tools and programs that align with the overall goals of the organization.
The value you’ll bring to the team:
- Responsible for development/implementation/maintenance and optimization of the Post-Acute Care Collaborative in the CFD including North, South and Polk.
- Responsible for development/deployment/evaluation of the Post-Acute Care Collaborative care management modes for transitions of care to post-acute.
- Establishes and/or maintains working and collaborative relationships with the healthcare providers (acute, and post-acute) community, as well as, strategic community healthcare relationships.
- Collaborates with Population Health Leaders in creating care pathways based on analytics and best practice standards for populations served.
- Leads in the design, implementation/evaluation of clinical related processes, policies and programs for the Division Post-Acute Care Collaborative to support standardization where appropriate in support of organizational goals.
- Responsible for regular communications through monthly collaborative meetings and in person site or virtual visit
QualificationsThe expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
- Bachelor’s degree in Nursing or other healthcare related field
- Five (5) years of recent post-acute healthcare experience
- Three (3) years of leadership experience
- Current, valid State of Florida license to practice professional nursing
