Duke Homecare & Hospice offers hospice, home health and infusion services as well as serves as the home for the Duke Caregiver Support Program. Our Hospice team works closely with a patientâ€™s physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham. Our team of nurses, therapists, social workers, counselors, and chaplains help patients and their families manage advanced medical conditions.
Duke Home Health provides services in a patientâ€™s home that include skilled nursing, physical therapy, occupational therapy, speech therapy, social service, home health aides, and pain and wound management. Our infusion team comes to a patientâ€™s home to administer intravenous medications, including antibiotics and chemotherapy, to children and adults. Our nurses, pharmacists, dietitians, and patient services coordinators work under the direction of a patientâ€™s doctor to monitor needs and treatment.
Duke HomeCare & Hospice Position Description
In addition to the responsibilities and requirements described in the Duke job description, the Duke HomeCare & Hospice Social Worker - Home Health, Hospice will be responsible for:
Provides psychosocial assessments, intervention, and /or treatment related to the patient's illness, and/or life situation. Identifies community resources to assist and meet the continuing needs of the patient and family in the home environment. May evaluate patients referred to the hospice program for admission
MAJOR JOB RESPONSIBILITIES:
Maintains up to date knowledge of Medicare/Medicaid/JCAHO regulations.
Identities personal learning needs and implements corrective action: plans and conducts staff development programs to improve agency effectiveness.
Provide psychosocial assessments of patients and family to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
Formulate, develop, and implement a plan utilizing appropriate social work interventions.
Collaborate with the health care team and involve the patient/family in the development and implementation of plans.
Provide education and counseling to patients and families around issues related to adaptation to the patient's illness and/or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies.
Maintain working knowledge of and liaison with community agencies; participate in area activities.
Assist with screening, identification and management of victims of abuse, neglect, domestic violence, rape, etc. Make referrals as appropriate and serves as resource for the health care team
May participate in on-call or after hours coverage.
Document findings, plan, and actions taken according to departmental guidelines and standards.
Participate and represent social work perspective in multidisciplinary teams, rounds, etc.
Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse, etc.
Serve as patient advocate.
Maintain records and statistics in accordance with department policies.
Attend and participate in staff, committee, department, and other administrative meetings.
Keep current with social work and health care developments and seek to increase further enhancement of job related knowledge.
Provide education to other health care professionals.
Maintains confidentiality of employee and patient information.
Exhibits an attitude which promotes harmony and goodwill in the workplace.
Displays a clean, neat, professional appearance.
Participates in Performance Improvement Program.
Promotes quality, comprehensive services through a team approach.
Performs other duties as assigned.
WORK ENVIRONMENT AND REQUIREMENTS:
Perform Patient care primarily in the patient's residence or skilled nursing facility (>50%)
Travel to and from patient residence
Risk exposure for this environment is high
Education and Formal Training
Bachelors in Social Work from CSWE accredited school of social work
Professional Work Experience
One or more years' experience in health care setting or social agency; can include social work internship
Knowledge, Skills & Abilities
Valid NC Drivers' License.
Working knowledge of Medicare and Medicaid regulations, including JCAHO and DFS standards, ICD-9 Coding.
Excellent written and verbal communication skills.
Strong interpersonal skills; ability to collaborate well.
Solid knowledge of social work practice: strong assessment, interview and counseling skills.
Strong knowledge of governmental and private organizations and community resources.
